How to Make a Claim
Freedom Healthnet staff are here to ensure your claim is dealt with swiftly and as hassle free as possible.
- The claims procedure will start at the time your General Practitioner refers you to a Specialist. You must tell your General Practitioner that you wish to have private treatment.
- Contact our Claims Helpline on 08703 50 40 30 immediately and a member of our claims team will take your details.
- Please have your policy number ready when you phone. Once your membership has been confirmed you will be sent a claim form. This is also available on our website and may download it. Claim Form PDF
- You will receive "Part A" of the claim form. Please have this completed by your General Practitioner, and return to us for pre-approval.
- We will then notify you whether or not the required consultation or treatment is covered by your Policy, by phone and/or in writing.
- Once you receive confirmation, see your chosen Specialist
- If your Specialist recommends further treatment or surgical procedure(s), we will forward you "Part B" of the claim form. Please have this completed by your chosen Specialist, confirming that the treatment or surgical procedure is medically necessary, and return to us for approval.
- We will then confirm the monetary amount your policy pays in respect of the claim and pay you direct either by cheque or by BACS.
Please note that benefits will be paid net of any excess agreed under the terms of the policy.
