Documentation Needed

With your Freedom cashplan health insurance policy or Freedom elite comprehensive policy, your General Practitioner must refer you to a Specialist and you may need to fill out a claim form.

Firstly, contact our Claims Helpline on 08703 50 40 30 and request a claim form to be sent direct to you. Alternatively, you can download a Claim form

 

The following sections will need to be fully competed and returned to our Claims Department:

1. Section 1-4 of the claim form completed by you.
2. Part A of the claim form to be fully completed by your General Practitioner
3. Part B of the claim form to be completed by your Consultant Specialist.

All these forms can be faxed to 08703 504040 to start the process, but please also post to:

Please note: we may also request further information such as a copy of your medical records from your General Practitioner or Specialist. We will always inform you if we need to do this.
