Documentation Needed

With your Freedom health insurance policy, once your General Practitioner has referred you to a Specialist you will need to contact our Claims Helpline on 08703 50 40 30 and request a claim form to be sent direct to you. Alternatively, you can download this form by clicking here.

The following sections will need to be fully competed and returned to our Claims Department:

  1. Section 1-4 of the claim form completed by you.
  2. Part A of the claim form to be fully completed by your General Practitioner
  3. Part B of the claim form to be completed by your Consultant Specialist.

All these forms can be faxed to 08703 504040 to start the process, but please also post to:

    Please note: we may also request further information such as a copy of your medical records from your General Practitioner or Specialist. We will always inform you if we need to do this.
